Submit Records Request Using a Paper Release Form

Please download and complete the Health Information Release Form. Email the form to ROI@parrishmed.com, or bring it to the Health Information Management Department located at Parrish Medical Center, 951 North Washington Avenue, Titusville, FL 32796. The office is open Monday to Friday from 8:30 a.m. to 5:00 p.m.

Birth and Death Certificate Requests

Birth and Death certificates are not available from Parrish Healthcare’s Health Information Management Department. These records must be ordered from the Florida Bureau of Vital Statistics.
